Who is this registration for?
WAIVER of NEGLIGENCE AND COMPLETE RELEASE OF LIABILITY
Tulsa Oktoberfest, Inc., for Zeeco Oktoberfest 2024
I wish to participate in Zeeco Oktoberfest 2024, which I understand to be a public festival staged at the River West Festival Park, and related events, including but not limited to “committee meetings”, “set-up and tear-down duties”, and other festival operations activities. I am aware that accidents can occur during such activities and that I may be injured as a result. I am voluntarily participating in the event with knowledge of the dangers involved and I agree to accept any and all risks. Oktoberfest volunteers are not employees of Tulsa Oktoberfest, Inc., nor the River Parks Authority, the City of Tulsa or Tulsa County. As such, Worker’s Compensation Insurance is not provided to me.
In consideration for being permitted to participate in Oktoberfest, I agree to assume all risks and to hold harmless Tulsa Oktoberfest Inc., its affiliated organizations, sponsors, officials, friends of Oktoberfest and all other government or public entities (and their respective officers, directors, agents, employees and members) who through negligence, carelessness or any other cause, might otherwise be liable to me for any claims, costs, risks and expenses including reasonable attorney fees. I am physically capable of participating in Oktoberfest. I acknowledge that I, and I alone, am solely responsible for my personal health and safety. I will abide by all rules and regulations established by Oktoberfest organizers and personnel as well as all City of Tulsa and Tulsa County regulations and codes, including usage of golf carts, mobile equipment and vehicles.
My name, photograph or likeness, and any property of mine may be used by Oktoberfest. I consent to such use in connection therewith.
I have carefully read this waiver and release and understand its contents. I certify I am signing of my own free will.

For minor volunteers, signature of parent/legal guardian will be required at Volunteer Check-in.
I hereby volunteer my services to this project under the management of Tulsa Oktoberfest, Inc. I understand that I am going to have fun, work hard, and make a difference! I understand there are some risks of physical injury involved in my volunteer assignment, and I will study, understand and avoid any and all dangers. I will not accept any work
assignment I feel I am not qualified or feel comfortable doing.
I agree to release Tulsa Oktoberfest, Inc. of any and all claims that may arise as a result of any
expenses, personal injury, loss or damages incurred while volunteering. I
understand that pictures of me taken while participating at this Conservancy event
may be used in promotional material such as social media (Facebook, e-‐newsletter,
and/or CRC’s website), brochures, and videos.
Parental Consent: (To participate in this project with Charles River Conservancy
your child must have your written permission) I will take the responsibility to see
that my child is properly prepared for all activities including: having the proper
clothing and shoes, and being in good health.
In the event that I cannot be reached in an EMERGENCY I hereby give my
permission to the physician selected by the Volunteer Coordinator to hospitalize
and secure proper treatment for my child as named above.
